Joint Commission Issues Sentinel Event Alert (SEA): Preventing Infection from Misuse of Vials

On June 16, the Joint Commission issued a SEA to raise awareness regarding the risk of infection due to misuse of single-dose or multi-dose vials. Since 2001, the CDC has documented at least 49 outbreaks that can be attributed to the “mishandling of injectable medical products.”  The alert describes the common underlying causes of such mishandling, such as lapses in basic infection control practice or a desire to prevent waste.
